To Our Patients
We appreciate your selection of this office to serve your health care needs. The providers and staff will do all we can to provide you with the best care. We feel that the more you know about our policies and methods of practice, the more we can be of service to you. Our practice is limited to sleep-related problems.  We see new patients preferably on physician referral.

Appointments may be made by calling 563-323-1352 between the hours of 8:30 am and 5:00 pm, Monday through Friday. If you feel your problem cannot wait until your scheduled appointment time, your call will be forwarded to the medical staff for better assessment of your problem, and appropriate steps can be taken. We will do our best to accommodate your schedule. We ask that you inform our office within 24 hours if you cannot keep your appointment

Our staff will be happy to discuss our fees with you. Our fees are based on our overhead, and the prevailing fees in the community. With inflation as it is, we are very sensitive about the cost of care and will do whatever possible to keep costs down. Our initial fee includes consultation and a review of your past medical records.  To minimize overhead expenses, payment is expected at time of service if your insurance is not one that we participate with.
COPAYS and DEDUCTIBLES ARE DUE AT TIME OF SERVICE. Our insurance representative is available to speak to explain our policies to you.

The physician/patient relationship is a privileged one. Your medical record is strictly confidential. No information will be made available to anyone without written authorization from you.


Insurance Information

Although we participate with most major insurances, frequent changes prevent us from listing each participating company on this website. Please feel free to call our office or your insurance company to check in network participation status. Because each insurance plan is different, your insurance carrier can also advise you about your particular benefits. Although we will file your insurance for you, payment is expected at the time of service for all insurances we do not participate with.  

If your insurance requires prior authorization or a referral from your primary care physician, please contact their office before your appointment with Mahadevia Sleep Consultants. If you need to discuss payment arrangements, please contact our insurance representative prior to your scheduled appointment at 563-323-1352.​
