What should I expect at my first appointment?
You have been referred to our office for a sleep problem. This may include, but not limited to, sleep apnea, insomnia, periodic limb movement, delayed sleep phase, narcolepsy or cataplexy. When you first arrive you will check in with the receptionist and they will collect all necessary documentation and information from you. This information includes your Photo ID, Insurance Cards, Medication List and the new patient paperwork listed above. After checking in with the receptionist, you will meet with the provider who will assess all of the information provided by you and a referring physician, if any. The provider will then discuss a treatment plan with you and what steps need to happen to help you sleep better.
